LA FOUND BRACELET DISTRIBUTION DECEMBER DATES

Through L.A. FOUND, if your loved one suffers from Alzheimer’s, dementia, autism or any cognitive impairment and is mobile, they may qualify for a free tracking bracelet. To obtain a free bracelet, you can sign up through Eventbrite (links below) and go to a location listed above OR call 1-833-569-7651. If you cannot attend, you can arrange to pick up the device from the L.A. FOUND office.
L.A. FOUND will be distributing free bracelets at the following dates, times and locations:

To get a free tracking bracelet, you must bring the following:
- Government issued identifications for both the participant and the authorized representative/caregiver
- Power of attorney, conservatorship or healthcare directive, if available
- Participant’s birth certificate if he/she is a minor
